Let us first begin with an example. Assume you have a list as follows in your text:

The code at the bottom will define a new function called renumber, which you can call using M-x renumber. Put point at the beginning of the paragraph and call it. This should change the number of the last line as follows:

You can also put point at the beginning of the second line and use M-x renumber to start from there:

And you can use the numeric prefix argument to start from any other number. Use C-u 3 M-x renumber at the beginning of the paragraph to get the following list:

Starting with emacs version >=22, it is now possible to execute arbitrary lisp code to generate the replacement string used by the
replace-regexp command. Let us renumber the first example given above using this new feature.
M-x replace-regexp RET [0-9]+ RET \,(1+ \#) RET
does the trick:

\,( ) executes the Lisp code in parenthesis and replaces the string matching the regexp by the output of that code. Several variables are available in the Lisp code:
\n returns the string matched by group number n in the regexp
\# returns the number of replacements done so far, starting at 0.
\& returns the whole string matched by the regexp
\? asks in the minibuffer for an explicit replacement
So, if you want to start numbering at 5, and only take into account numbers at the beginning of each line, you can use:
M-x replace-regexp RET ^[0-9]+ RET \,(+ 5 (string-to-int \#)) RET
query-replace-regexp if you want to be asked whether to do the replacement for each match. Note that
replace-regexp operates on the selected region if transient mark mode is enabled. If transient-mark-mode is not enabled, it starts at point and proceeds until end of buffer. You can enable transient-mark-mode for the next command just by typing
